This is a 6 month, 37.5 hours per week, position with the possibility of extending after 6 months.


Job Description Summary/Objective

Caregiver Coordinator is responsible for the delivery of information and assistance and options counseling to individuals and their caregivers in order to provide support and assistance in their caregiving role. Coordinator delivers person-centered counseling that supports individuals and their caregiver(s) in their deliberations to make choices regarding their short and long-term needs/goals. Coordinator provides community education and outreach in order to raise awareness of JABA services. Additionally they will develop resources, training and support groups focused on addressing caregiver needs.

Essential Functions

Service Delivery: Support individuals in the development of a plan of care designed to meet their short and/or long-term goals for care based on their preferences. Engage caregivers and other supports in the development and execution of a person-centered plan to optimize their ability to live in their environment of choice. Locate potential resources to meet the identified needs; develop a thorough working knowledge of community and statewide resources to include public benefit programs. Organize and lead caregiver support groups.

Data Management and Reporting: Ensure timely and accurate entry of activities into required client data management systems. Monthly reporting of advocacy activities and professional development. Monthly reporting of survey data for performance measurement.

Resource Information Maintenance: Create and maintain accurate information on community services and supports as well as supportive online resources such as apps, toolkits, support groups, videos, etc. Maintain updated resource information; utilize statewide databases such as VirginiaNavigator.

Community Education/Advocacy: Educate individuals and the general public about JABA services and other community resources and services in order to improve access and understanding of program. To perform duties in an ethical and professional manner as representative of JABA.

Certifications: Certification as an Options Counselor within first month of hire. May also be required to seek additional training and certifications as part of employee and program development.

Maintaining Confidentiality: Commit to the protection of confidential information related to the agency, its operation, proprietary documents, financials, and the individuals served. Comply with data integrity and security policies.

Professional Development: Attend mandatory meetings and trainings as determined by supervisor. Complete at a minimum of 10 hours annually of professional development to improve skills and knowledge.

Work Environment

Employee works primarily in an office environment, this position also requires, the employee travels by automobile and is exposed to changing weather conditions. The employee works in varied residential environments based on assigned caseload.

Telecommuting may be permitted on the decision of supervising manager after probation has been completed and employee demonstrates strong self-management skills. Based on the needs of the position, this opportunity may be suspended at times. Employees are required to have a dedicated work area established that is separated from other living areas and provides information privacy and the ability to keep all company sensitive documents secure (if applicable).

• This is a largely sedentary role with periods of moderate activity.

• The employee is regularly required to participate in online and in person meetings and communicate effectively with team members, peers, and community partners.

• Employee uses a computer extensively for data entry and other functions of the position.

• Employee expected to maintain physical files and filing system.

• The position works from an office or telecommuting environment and routinely travels to other locations locally and within Virginia.

• Employee also routinely conducts assessments within client homes which are most often not accessible by wheelchair.

• On occasion, the employee will represent JABA at community events to distribute materials and discuss JABA programming.

This is a full-time position working 37.5 hours, Monday – Friday from 8:30AM to 5:00PM
