This is a Part Time position (20 hours per week) working at out Greene County Center.

This is a professional position which provides health screenings and nursing services to clients at JABA community centers and other clinic locations and homebound clients in Planning District Ten. The registered nurse provides health assessments, develops and implements nursing care plans/clinical pathways to assist clients towards optimum levels of health independence and self-care, fulfilling these duties in collaboration with caregivers where appropriate. Position also coordinates with clients’ primary care physicians and works as part of an interdisciplinary team, collaborating with Aging Services Coordinators and other JABA service providers.

1. Provide nursing services to clients served by the JABA Health Services program at assigned health service sites and make home visits in the related geographical areas for the assigned hours per week.

2. Provide supervision/consultation to the assigned JABA community centers.

3. Perform data collection and completion of the Universal Assessment Instrument (UAI), daily summary sheets, and medical information needed for the client medical record.

4. Provide regular follow-up of client’s medication management needs related to acute and chronic health issues; focused counseling and referral as indicated by findings; health promotion education including disease prevention, medication box management, adult immunizations and alternative and complimentary therapies appropriate to population.

Health Education

1. Obtain and maintain Red Cross Instructor Certification in CPR, AED, and First Aid.

2. Provide Red Cross Certification in CPR, AED, and First Aid to JABA Staff.

3. Obtain and Maintain Chronic Disease Self-Management Education Leader Certification.

4. Lead Chronic Disease Self-Management Education Classes in person, virtually, and via phone.

    5. Work with At Home with JABA as directed
