This is a Part time position working 17.5 hours per week
The Resource Counselor assists adults over 60, adults over 18 with disabilities and/or caretakers who need assistance with service providers as well as providing information about agencies and organizations that offer services.
1. Service Delivery: Responding to requests for information about short- and long-term services and supports available in PD 10. Conducting initial intake making internal and external referrals. Determining the Need for Options Counselors and determining appropriate staff assignment on referrals to Options Counseling program. Assisting with other programs on an as-needed basis as directed by supervisor.
2. Documentation and record-keeping: Assuring client records are maintained and documenting all interactions with individuals and caregivers in NWD (No Wrong Door) tool. Conducting follow-up surveys with assisted clients, and/or supervising volunteer conducting follow-up surveys
Local travel may be required.
